Corewell Health's Visiting Nurses Association in West-Central Michigan is hiring a Licensed Practical Nurse for home health in the Holland and Zeeland areas, with a part-time schedule and a $7,500 sign-on bonus for eligible new LPNs.

• Documents care provided via computer/written patient record in a manner consistent with guidelines.
• Greets and communicates with patients and families with care and respect, coordinating with the health care team for continuity of care.
• Cleans patient's environment/work area/equipment and communicates any equipment malfunctions.
• Documents patient chargeable items according to standards.
• Participates in selected quality monitoring activities and maintains ethical standards.

Job Summary

Our Visiting Nurses Association is our Home Care Skilled Nursing Team for West-Central areas of Michigan. We offer flexible work schedules including working every 4th weekend, working every 3rd holiday and on call 1-2 days in a 6 week rotation. For experienced LPN’s this is an autonomous position that drives you to use all of your top nursing skills and critical thinking. Working in home health enables you to treat the patient in the comfort of their home, where patients are able to thrive and own their own health care. We create personalized assignments close to home that enable our nurses to visit between 5-7 patients per shift. From Ludington, to Baldwin, to Nunica and south through Kent County, Ottawa County and as far south as Gun Lake, our Corewell Health West, Visiting Nurses Team goes the distance to provide outstanding patient care

Scope of work

Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care. Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician. Documents care provided via computer/written patient record in a manner consistent with guidelines.

Essential Functions

• Collects and monitors assessment data and reports physiological, psychological, sociocultural and spiritual needs of patients and families in coordination with and as delineated by the RN/Mid-level Provider/Physician.
• Provides direct patient care under the direction of the RN/Mid-level Provider/Physician, consistent with policies and procedures, and unit based routines.
• Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician.
• Documents care provided via computer/written patient record in a manner consistent with guidelines.
• Greets and communicates with patients and families in a manner consistent with caring and respect. Communicates and collaborates with other members of the health care team in order to ensure continuity of care and coordination of service.
• Cleans patient's environment/work area/equipment and communicates any equipment malfunctions to appropriate department/unit personnel.
• Documents patient chargeable items according to standards.
• Participates in selected quality monitoring activities under the direction of the RN / Mid-level Provider / Physician. Demonstrates a high standard of moral and ethical behavior. Recognizes appropriate and ethical behaviors and identifies resources for resolution as needed. Advocates for privacy, confidentiality, and security of patient, staff and organization data.
