Job summary

Corewell Health West Visiting Nurses Association provides home care nursing across West-Central Michigan, including Big Rapids and Evart. We welcome experienced RNs, LPNs, and new grads to deliver autonomous, patient-centered care in patients’ homes. Position emphasizes clinical judgment, compliance with BLS, and collaboration with the care team.

Qualifications

• Associates in Nursing required.
• Bachelor's in Nursing required within 5 years.

• AHA or ARC BLS required within 90 days.

Responsibilities

• Assess, monitor, plan, and implement the treatment plan under Nursing Manager supervision.
• Apply clinical judgment to prevent physiological complications.
• Collaborate with the interdisciplinary team to support patient care.
• Provide home-based nursing visits to patients in Big Rapids/Evart areas.

This position will service the Big Rapids and Evart areas of Home Care.

About the unit

Our Visiting Nurses Association is our Home Care Skilled Nursing Team for West-Central areas of Michigan. We are accepting applications from experienced RN’s, LPN’s, and New Grads! For experienced RN’s this is an autonomous position that drives you to use all of your top nursing skills and critical thinking. Working in home health enables you to treat the patient in the comfort of their home, where patients are able to thrive and own their own health care. We create personalized assignments close to home that enable our nurses to visit between 5-7 patients per shift. From Ludington, to Baldwin, to Nunica and south through Kent County, Ottawa County and as far south as Gun Lake, our Corewell Health West, Visiting Nurses Team goes the distance to provide outstanding patient care

Scope of work

Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
