Job summary

Loyola Medicine Home Care and Hospice is seeking an experienced LPN to deliver compassionate, one-to-one care to patients in their homes. You will assess, plan and implement care under physician orders while adhering to licensure requirements.

Qualifications

• Graduate of an accredited or state approved school of practical nursing.
• Licensed Practical Nurse (LPN) in Illinois.
• Minimum 2–3 years experience in acute care; home care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passed NAPNES or equivalent medication exam before administering medications.

Responsibilities

• Provide one-to-one care and monitor patients in their homes.
• Perform patient assessments, develop care plans, and supervise outcomes.
• Follow physician orders and comply with licensure requirements and policies.
• Utilize Home Care Connect virtual care program and remote monitoring.
• Collaborate with RNs and healthcare team to ensure quality outcomes.

Our nurses follow physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.

Minimum Qualifications

• Graduate of an accredited or state approved school of practical nursing
• Licensure as a LPN in the State of Illinois
• A minimum of 2-3 years of experience in acute care setting; home care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
