Mass General Brigham Home Care, Inc. is seeking licensed practical nurses to deliver compassionate, one-on-one home care across multiple communities including Chelsea.

The role offers hybrid scheduling, flexible shifts, and opportunities to build lasting patient relationships while maintaining clinical autonomy. Required are an active Massachusetts LPN license and at least 1 year of acute/sub-acute nursing experience; home care experience is preferred.

• Active Massachusetts LPN license.
• Minimum 1 year of acute or sub-acute nursing experience.
• Home care experience preferred.
• Strong clinical, communication, and organizational skills.
• Comfortable with electronic documentation and mobile technology.
