UVM Home Health, Inc. is offering full-time, part-time, and per diem LPN opportunities in Middlebury, Vermont. This role includes a sign-on bonus, shift differentials, and a supportive environment focused on providing quality long-term patient care within skilled nursing units. Ideal candidates demonstrate empathy, reliability, and strong communication skills.
We have a variety of full-time, part-time, and per diem LPN opportunities on all three of our nursing units within our Skilled Nursing Facility. Post-acute is currently not looking for per diem applicants.
For full and part-time LPN positions, we offer a sign-on bonus of up to $7,500, pro-rated for part-time and based on years of experience.
We offer the following shift differentials on top of the hourly rate of pay:

Experience:
Previous long-term care experience is preferred but not required. The ideal candidate will be empathetic, compassionate, reliable, and a team player. Excellent communication skills and basic computer skills are necessary. Must have a high aptitude for learning and patient care, along with strong customer service skills.
Helen Porter Rehabilitation & Nursing is a welcoming, secure residence offering skilled nursing care, assistance with daily living, privacy, and respect for individuality. Located in Middlebury's farmlands, it accommodates 98 residents, is licensed by Vermont, and certified by Medicare and Medicaid. It provides a home-like environment with specialized care services.

Long-Term Care Unit: Offering high-quality long-term care, separate from Memory Care and Short-Term Post-Acute Rehabilitation, with personalized programs and activities.
Memory Care Unit: Designed for individuals with dementia, focusing on activity-based care in a safe, supportive environment to maintain dignity and purpose. Staffed with dedicated advocates and team members.
Post-Acute Care: Provides supportive, goal-directed care after surgery or hospitalization, including conditions like strokes, neurological, orthopedic, respiratory, cardiac, and post-trauma cases. Therapy services are available Monday through Saturday, with nursing staff supporting therapy activities when therapists are not present.
