Job summary

Responsibilities

• Monitor residents and communicate changes in condition.
• Complete accurate and timely documentation in MatrixCare.
• Communicate with physicians, families, and the interdisciplinary team.
• Supervise and support CNAs on the unit.
• Help maintain a safe, comfortable, and positive environment for residents.
