Job summary
FellowshipLIFE is seeking a dedicated Licensed Practical Nurse (LPN) in Hackettstown, NJ. This full-time role focuses on direct patient care under supervision, medication administration, and collaborative care for aging adults in our Life Plan Communities.
The ideal candidate will hold a New Jersey LPN license with at least 2 years of experience, strong geriatric nursing skills, and CPR/BLS certifications. Join a mission-driven team that values growth, compassion, and excellence.
Qualifications
- Valid New Jersey LPN license required.
- Minimum of 2 years of experience in nursing.
- Strong geriatric nursing knowledge, especially sub-acute care and assisted living.
- CPR and BLS certifications required.
Responsibilities
- Provide direct patient care under supervision of RNs or physicians.
- Administer medications and treatments per orders.
- Monitor vital signs and assess patient condition.
- Develop and implement patient care plans.
- Document patient information, treatments and responses.
- Collaborate with other healthcare professionals for coordinated care.
- Maintain a clean and safe patient environment.
- Adhere to nursing policies, procedures and protocols.
- Respond promptly to emergencies and provide interventions.
