Job summary

VIVA USA Inc. in Phoenix, AZ is seeking a licensed practical nurse (LPN) to join our on-site team. You will administer medications and treatments on an assigned unit as ordered by the physician and within your scope of license. Schedule: 12-hour shifts, 6:00am–6:30pm, full time.

You will observe and report the effectiveness of therapies, maintain clinical and administrative documentation, and participate in QAPI and interdisciplinary care planning to support resident safety and quality of life.

Qualifications

• Active LPN license in Arizona is required.
• Ability to work within a team of healthcare professionals.
• Knowledge of geriatric nursing practices and patient care plans.

Responsibilities

• Administer medications and treatments on an assigned unit as ordered by the physician and within scope of license.
• Observe and report the effectiveness of medications and treatments.
• Maintain clinical and administrative documentation regarding diagnosis, observations, and progress.
• Participate in shift reports and interdisciplinary team communication.
• Attend staff meetings and participate in quality and performance improvement activities.
