Qualifications

• Current, unencumbered active LPN license in Massachusetts.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with standards and plan of care.
• Follow and implement individualized care plans by Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per plan and policies.
• Monitor residents’ conditions and report changes promptly to supervising staff.
• Maintain accurate, timely documentation per orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.
