Job summary

Home Health Care, Inc. seeks an Licensed Practical Nurse (LPN) to provide patient-centered nursing care in a facility setting within the LPN scope of practice. You will administer medications and treatments, monitor patient conditions, and document changes.

Responsibilities include wound care, patient education, ADLs, coordination with the healthcare team, and reporting updates to ensure quality care. A valid LPN license and BLS/CPR are required, with at least 1 year of facility-based experience.

Qualifications
