Job summary
David Raines Community Health Center in Shreveport, LA is seeking a Licensed Practical Nurse to deliver essential nursing care under the supervision of RNs, NPs, or physicians. The LPN will assess patients, administer medications, and help maintain a safe, organized clinic environment.
The position requires a Louisiana LPN license and BLS; experience with electronic health records (NextGen or similar) is preferred. We value teamwork and patient-centered care within the SBHC calendar year.
Qualifications
- Graduate from an accredited school of practical nursing.
- Must have Louisiana LPN/VN license and current credentialing.
- BLS certification required (American Heart Association).
- Experience with electronic health records preferred (NextGen or similar).
Responsibilities
- Obtain vital signs and assist with specimen collection as ordered.
- Notify physician of urgent care needs and changes in patient condition.
- Administer medications per physician orders and policy.
- Prepare patients for examinations and maintain clean, safe exam rooms.
- Transcribe orders and document information in medical records.
- Assist with clerical duties as needed.
