Job summary

Easterseals PORT Health is seeking a Licensed Practical Nurse (LPN) to join the Virginia REACH Adult Crisis Therapeutic Home (ACTH) team in Woodbridge, Virginia. This on-site, part-time role focuses on direct nursing care for adults with I/DD and co-occurring behavioral health needs in a crisis stabilization setting.

Qualifications

• Current, unrestricted Virginia LPN license.
• Experience providing care in a crisis, residential, or behavioral health setting.
• Strong verbal communication, documentation, and data-entry skills.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide nursing services and medical support to adults with I/DD in crisis stabilization.
• Administer medications safely and accurately in accordance with physician orders and clinical protocols.
• Conduct nursing assessments and monitor physical and behavioral health needs.
• Educate individuals regarding their health needs and empower them.
• Collaborate with interdisciplinary team to support treatment and recovery goals.
• Maintain accurate and timely clinical documentation.
