Job summary

Villa Maria Rehabilitation & Nursing in Plainfield, CT is seeking a dedicated LPN Floor Nurse / Charge Nurse to oversee nursing care on the unit. You will work under the RN Supervisor, ensure quality care, and support staff in a fast-paced setting.

Qualifications

• Valid Connecticut LPN or RN license in good standing.
• Minimum 1 year in long-term care or skilled nursing preferred.
• Strong clinical assessment, planning, implementation, evaluation.
• Proficient with PCC and Microsoft Office preferred.
• Excellent communication and leadership skills.
• Compassionate, dependable, and able to manage stress.

Responsibilities

• Accurately record and maintain residents’ medical histories, progress notes, and treatment records.
• Administer medications and treatments as ordered by physicians and according to facility protocols.
• Observe and monitor residents’ conditions, behaviors, and responses; report changes promptly.
• Monitor residents’ fluid intake and output and document observations.
• Administer injections, dress wounds, collect lab specimens, and perform direct care tasks as assigned.
• Supervise and support CNAs and other nursing staff to ensure safe, compassionate care.
• Provide emotional and psychological support to residents and families.
• Communicate with families, education, comfort, and updates on care plans.
• Ensure compliance with infection control, safety, and confidentiality policies.
• Maintain accurate charting in PCC and other electronic records systems.
• Exhibit professionalism, teamwork, and compassion in all interactions.
