Job summary

Villa Maria Rehabilitation & Nursing in Plainfield, CT seeks an experienced LPN Floor Nurse / Charge Nurse to provide direct resident care and supervise unit staff under the RN Supervisor. This full-time role is based at a long-term care facility emphasizing quality, person-centered care.

The successful candidate will have a CT license, at least 1 year of LTC experience, strong clinical skills, and excellent communication.

Qualifications

• Valid Connecticut LPN or RN license in good standing.
• Minimum of 1 year of experience in a long-term care or skilled nursing setting preferred.
• Strong clinical skills with the ability to assess, plan, implement, and evaluate resident care.
• Working knowledge of Microsoft Office and PointClickCare (PCC) preferred.
• Excellent communication and leadership skills.
• Compassionate, dependable, and able to manage stress effectively in a fast-paced environment.

Responsibilities

• Record and maintain residents’ medical histories, progress notes, and treatment records.
• Administer medications and treatments as ordered by physicians and according to facility protocols.
• Observe and monitor residents’ conditions, behaviors, and responses to medications or treatments; promptly report changes to the supervising RN and physician.
• Monitor residents’ fluid intake and output and document all observations accurately.
• Administer injections, dress wounds, collect lab specimens, and perform other direct care tasks as assigned.
• Supervise and support assigned CNAs and other nursing personnel to ensure compassionate, safe, and efficient care delivery.
• Provide emotional and psychological support to residents and their families.
• Communicate effectively with residents’ family members, providing education, comfort, and updates on care plans as appropriate.
• Ensure compliance with infection control, safety, and confidentiality policies.
• Maintain accurate and timely charting in PointClickCare (PCC) and other electronic record systems.
• Exhibit professionalism, teamwork, and compassion in all interactions.
