Job summary

Villa Maria Rehabilitation & Nursing in Plainfield, CT, seeks a dedicated LPN Floor Nurse / Charge Nurse to provide direct resident care and supervise unit staff. This full-time role reports to the RN Supervisor and aligns with facility policies and regulatory standards.

The ideal candidate holds a CT LPN or RN license, has at least 1 year in long-term care, and demonstrates strong leadership, communication, and clinical skills for a fast-paced environment.

Qualifications

• Valid Connecticut LPN or RN license in good standing.
• Minimum of 1 year of experience in a long-term care or skilled nursing setting preferred.
• Working knowledge of PCC (PointClickCare) and Microsoft Office.

Responsibilities

• Accurately record and maintain residents’ medical histories, progress notes, and treatment records.
• Administer medications and treatments as ordered by physicians and per facility protocols.
• Observe and report changes in residents’ conditions to the supervising RN and physician.
• Monitor residents’ fluid intake and output and document observations.
• Administer injections, dress wounds, collect lab specimens, and perform other direct care tasks.
• Supervise and support CNAs and other staff to ensure safe, compassionate care.
• Provide emotional and psychological support to residents and families.
• Communicate with families about care plans and updates.
• Ensure infection control, safety, and confidentiality policies are followed.
• Maintain charting in PCC and other electronic records.
• Demonstrate professionalism, teamwork, and compassion.
