Fieldstone Nursing and Rehabilitation Center is seeking a dedicated Licensed Practical Nurse to join our team in Ohio. You will provide direct patient care, administer medications, and work with an interdisciplinary team to ensure quality outcomes.

Qualifications

• LPN license in state of practice
• Basic Life Support (BLS) certification
• Basic understanding of medical terminology and anatomy
• Ability to lift, push, and pull equipment and supplies

Responsibilities

• Provide direct patient care, including administering medications, performing treatments, and documenting patient information
• Collaborate with other healthcare professionals to develop and implement individualized patient care plans
• Monitor and report patient condition, including any changes or concerns
• Participate in shift reports, patient care conferences, and other meetings as needed
• Maintain a clean and organized workspace, adhering to infection control and safety standards
• Other duties as assigned
