Job summary

Discovery Management Group is seeking a Licensed Practical Nurse (LPN) to provide compassionate nursing care in a senior living community in Hamilton, NJ. You will support resident safety, dignity, and well-being, following individualized care plans and collaborating with an interdisciplinary team.

The role offers multiple schedules (FT/PT/PRN) with weekend options, starting pay at $34/hour and up, and a comprehensive benefits package.

Qualifications

• Current Licensed Practical Nurse (LPN) license.
• Minimum of two (2) years of Licensed Nurse experience required.
• Experience in assisted living, long-term care, or a skilled nursing environment preferred.

Responsibilities

• Provide nursing care to residents per individualized care plans and policies.
• Conduct ongoing assessments and monitor changes in condition.
• Administer medications and treatments as prescribed and document.
• Coordinate care with physicians, families, pharmacies, and other providers.
• Support health promotion, wellness initiatives, and accurate clinical documentation.
• Assist with care planning and resident evaluations as required.
• Collaborate with caregivers and leadership to ensure high-quality care.
