Blountstown Center for Rehabilitation and Healing in Blountstown, FL is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in a supportive environment. The role includes admissions and transfers, medication management, and close collaboration with an interdisciplinary team to deliver optimal resident outcomes.

Qualifications

• Licensed Practical Nurse (LPN) licensed by the State of Florida.
• CPR certification required.
• Two years prior nursing experience preferred.

Responsibilities

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control, emergencies, and other center policies.
• Collaborate with interdisciplinary team for best outcomes.
• Report abnormal changes in residents’ condition.
• Provide nursing care for long-term or rehabilitative care.
• Collect patient data, observations, and relevant information.
• Implement and evaluate patient care plans as required by state regulations.
• Supervise care provided by other nursing personnel in certain situations.
• Maintain documentation requirements and equipment knowledge.
