Job summary

Blountstown Center for Rehabilitation and Healing in Blountstown, FL is seeking a Licensed Practical Nurse (LPN) to join our compassionate care team. The role focuses on delivering patient-centered nursing care in a supportive environment for long-term and short-term residents.

As an LPN you will administer medications, assist with admissions, collaborate with the interdisciplinary team, and ensure infection control and safety policies are followed.

Qualifications

• LPN (Licensed Practical Nurse) licensed by the State.
• CPR Certification required.
• Two Years prior nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Skills

Two Years nursing experience preferred
