You will provide nursing care for residents receiving long-term or rehabilitative services, under supervision, and collaborate with an interdisciplinary team to support patient outcomes.

Qualifications include an active LPN license, CPR certification, and 2 years of nursing experience preferred.

Qualifications

• LPN license valid in the state.
• CPR certification required.
• Two years nursing experience preferred.

Responsibilities

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Following infection control, emergencies, and other center policies.
• Collaborating with resident’s interdisciplinary team to provide the best outcomes.
• Identifying and reporting all abnormal changes in residents’ condition.
• Delivers nursing care for long-term or rehabilitative residents.
• Maintains documentation and follows center policies.
