Job summary

Orchard View Post-Acute and Rehabilitation Center in Kingsport, TN seeks an LPN to provide nursing care and supervise care teams, coordinating with the interdisciplinary team to ensure high-quality resident care.

The role involves direct patient care, supervision of CNAs/CMAs, accurate documentation, and participation in care planning, with a commitment incentive program available for the first year.

Qualifications

• High school diploma or equivalent is required.
• Current, unencumbered LPN license in Tennessee is required.
• Long-term care/skilled nursing experience is preferred.
• Strong assessment, critical thinking, and communication skills are essential.

Responsibilities

• Provide direct nursing care including treatments, wound care, and medication administration per orders and scope of practice.
• Supervise and support CNAs/CMAs, delegate tasks, and ensure completion of care.
• Monitor resident conditions, complete assessments, and report significant changes to the RN/provider.
• Participate in care planning, documentation, and resident/family education as needed.
• Ensure accurate and timely documentation in the medical record and logs.
• Promote infection control, safety, and regulatory compliance on the unit.
• Foster a positive, team-oriented environment focused on resident outcomes and satisfaction.
