Wise Medical Staffing is seeking experienced Licensed Practical Nurses (LPNs) for PRN and contract opportunities in a Long-Term Care facility in Navarre, Ohio. This role offers competitive pay, flexible scheduling, and the opportunity to deliver compassionate patient care.

Qualifications

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Active and unrestricted Pennsylvania LPN license required.
• Minimum 6 months of LPN experience required.
• Current BLS/CPR certification required.
• Annual physical required where applicable.
• Negative TB test required.
• COVID-19 vaccination or facility-specific requirements may be required.
• Ability to lift up to 80 pounds.

Responsibilities

• Provide skilled nursing care in a Long-Term Care setting.
• Admit, transfer, and discharge residents as appropriate.
• Monitor, assess, and report changes in resident condition.
• Administer medications and treatments as ordered.
• Perform wound care and collect laboratory specimens as needed.
• Communicate effectively with residents, families, and the healthcare team.
• Maintain accurate and timely clinical documentation.
• Restock medications, supplies, and equipment as needed.
• Collaborate with physicians and interdisciplinary healthcare teams.
• Follow facility policies, procedures, and patient safety standards.
