Job summary

DFW Home Health, part of the Optum family, is hiring a full-time LVN to join our Arlington-based team serving the Burleson area in Texas. The Licensed Vocational Nurse will provide in-home patient care, coordinate with the care team, and document visits per regulatory standards.

Qualified candidates hold current LPN licensure in Texas, CPR certification (or willing to obtain within 90 days), and have reliable transportation.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Current CPR certification or ability to complete within 90 days of hire
• Home care experience
• Ability to manage multiple tasks simultaneously
• Able to work independently
• Good communication, writing, and organizational skills
• Ability to work flexible hours as required to meet identified client needs

Responsibilities

• Provides high quality clinical services within the scope of practice in accordance with the plan of care.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, communicating with patient, family, physicians, other clinicians and case managers.
• Function in diverse home situations regardless of age, race, creed, color, sex, disability, or financial condition.
