Job summary

DFW Home Health, part of the UnitedHealth Group family, is hiring a full-time LVN/LPN to join our dedicated team in Arlington, Texas, serving the Mansfield area. The role focuses on delivering high-quality nursing care in patients' homes, documenting visits, and coordinating with a multidisciplinary team.

The position requires an active LPN license, a valid driver's license, and flexibility to work in various home settings.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Home care experience and ability to manage multiple tasks while working independently and communicating effectively.

Responsibilities

• Provide high quality clinical services within scope of practice per plan of care and infection control procedures.
• Document visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Coordinate care, report patient needs promptly, and communicate with patients, families, and care team.
• Function in diverse home environments and with diverse patient populations.
