Qualifications

• Must be currently licensed as an LPN in Pennsylvania.
• CPR Certification required.
• Experience in rehabilitative or geriatric nursing preferred.
• Ability to build rapport with patients and care teams.

Responsibilities

• Perform direct resident assessment and participate in care planning.
• Administer medications and treatments as prescribed.
• Communicate resident data to the appropriate party.
• Supervise and coordinate CNAs in delivering direct care.
• Maintain a compassionate, resident-focused care environment.
• Collaborate with team to ensure high-quality service.
