Job summary

PASCO/SW Home Health & Hospice seeks a Licensed Practical Nurse in Colorado to provide hands-on patient care under RN supervision in home settings. Colorado LPN/LVN license, CPR, and reliable transportation are required; 3 years nursing experience preferred, especially in Home Health or Med-Surg.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in Colorado.
• 3 years of nursing experience preferred (Home Health or Med-Surg a plus).
• Current CPR certification and reliable transportation.

Responsibilities

• Deliver compassionate, direct patient care in accordance with state regulations.
• Implement care plans developed by the RN and document progress accurately.
• Monitor patient conditions and communicate observations to the RN and physician.
• Collaborate with the care team to ensure seamless, high-quality service.
• Participate in professional development and team meetings.
