The Pennant Group in Grand Junction, CO, is seeking a dedicated Licensed Practical Nurse (LPN) to provide in-home patient care under RN supervision. You will help with comfort, safety, and outcomes in patients’ homes, following care plans and state regulations.

Qualificações

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in the State of Colorado.
• 3 years of nursing experience preferred (Home Health or Med-Surg a plus).
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsabilidades

• Deliver compassionate, direct patient care in home health settings.
• Follow care plans, document progress, and communicate with RN/physician.
• Monitor patient conditions and report observations to the RN and physician.
• Collaborate with care team for high-quality, coordinated service.
• Engage in professional development and team meetings.
