Empath Health in Bradenton, FL is seeking a dedicated LPN to join our Field ALF/LTC North team. Under the supervision of the RN case manager, you will provide bedside care, administer prescribed treatments, and offer physical and emotional support to patients while reporting changes as needed.

You will collaborate to process physician orders, monitor patient comfort, and educate families, with a minimum of one year acute-care experience and Florida LPN licensure; hospice LPN experience is

Qualificações

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.

Responsabilidades

• Under the direction of the RN case manager, provides continuous bedside clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• The LPN utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the Patient/Family unit.
