Job summary

ThedaCare is seeking an Ambulatory Licensed Practical Nurse in Appleton to join our team. This 0.8 FTE position provides patient-centered nursing care across the ambulatory continuum under supervision of a registered nurse or physician.

Schedule: Monday–Friday, 7:30 AM–5:00 PM with no weekends or holidays. The role emphasizes care coordination, documentation in the EHR, and collaboration with an interprofessional team to improve outcomes and the patient experience.

Qualifications

• Current Wisconsin LPN licensure.
• American Heart Association BLS certification.
• Ability to lift up to 50 pounds and move patients as needed.

Responsibilities

• Perform delegated tasks using evidence-based practice to assess patient conditions.
• Collaborate with interprofessional team to meet patient care needs.
• Document care and ensure accurate medication reconciliation in the EHR.
• Support patient education and safety during office visits and procedures.
• Assist in discharge planning and follow-up instructions with patients.
• Maintain professional relationships with care team to optimize outcomes.
