Job summary

Under supervision of RNs, APPs, or physicians, you will perform routine tasks, support care transitions, document in the EHR, and contribute to safe, high-quality outcomes while maintaining professionalism and compassion.

Qualifications

• Current Wisconsin LPN licensure.
• American Heart Association BLS certification.
• Additional departmental certifications may be required.

Responsibilities

• Performs delegated tasks and collects data on patient condition.
• Collaborates with care team to meet patient needs and outcomes.
• Provides age-appropriate, culturally sensitive nursing care within scope of practice.
• Documents medication reconciliation and patient information in EHR.
• Supports patient education and discharge planning.
