Job summary

This role involves collaborating with the care team, data collection, medication management, and patient education, with a 40-hour workweek and a 7 am–6 pm schedule in an ambulatory setting.

Qualifications

• Current Wisconsin LPN Licensure.
• American Heart Association Healthcare Provider Basic Life Support (BLS)

Responsibilities

• Performs delegated tasks using evidence-based practice to collect data on patients and assists with the plan of care.
• Collaborates with interprofessional team to meet patient needs and improve outcomes.
• Documents nursing care and prepares patients for examinations.
• Manages medication refills, prior authorizations, and forms within scope of licensure.
• Reinforces patient education to improve care outcomes and safe practices.
• Assists in discharge planning and coordinating follow-up care.
