Job summary

West Park Rehabilitation and Nursing Center is hiring Licensed Practical Nurses (LPNs) to provide direct nursing care to residents. We value compassion, teamwork, and a commitment to high-quality care in a safe, supportive setting.

The role involves administering medications, monitoring health, and collaborating with physicians and RNs. Weekend availability and 8-hour shifts are required, with day, night, and overnight options, in a rehabilitation and long-term care environment.

Qualifications

• Licensed Practical Nurse (LPN) license.
• Compassionate, patient-focused and committed to high-quality care.
• Ability to work independently and collaboratively within a team.
• Excellent communication and interpersonal skills.
• Ability to maintain accurate records and handle sensitive information confidentially.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health conditions, including vital signs, symptoms, and responses to treatments.
• Assist in the development and implementation of individualized care plans.
• Provide nursing care for residents, including wound care, catheter care, and assistance with daily living activities.
• Collaborate with physicians, RNs, and other healthcare team members to ensure coordinated and comprehensive care.
• Educate residents and their families about health management and procedures.
• Ensure that all care is delivered in accordance with regulatory and facility guidelines.
• Respond to emergencies and provide critical care when necessary.
• Assist with training and supervising nursing assistants (CNA).
