The Licensed Practical Nurse (LPN) provides professional nursing care for residents in long-term care under the supervision of a registered nurse or physician. This role includes administering medications, following care plans, documenting care, and collaborating with the interdisciplinary team to ensure resident safety and comfort.

Qualifications

• Graduate from an accredited practical nursing program.
• Current LPN license with state nursing board.
• Associate degree in practical nursing preferred.

Responsibilities

• Provide nursing care to residents under supervision of a RN/physician.
• Administer medications and treatments as ordered; monitor and respond to resident status.
• Document resident care and outcomes; communicate with the care team and families as appropriate.
