Job summary

Springbrook NY Inc. seeks an LPN to provide comprehensive nursing care under the supervision of a registered nurse. You will assist individuals with daily activities, document care, and participate in medical appointments as part of a collaborative care team.

Responsibilities include medication administration, wound care, and following physician-approved orders. Candidates must hold an NYS LPN license and prefer experience with developmental disabilities.

Qualifications

• Graduate of an approved practical nursing program and current NYS LPN license.
• Two years’ experience with individuals with developmental disabilities preferred.
• Proficiency with common computer software and email apps (Excel/Outlook/Word).

Responsibilities

• Provide nursing care under RN direction for individuals’ health needs.
• Assist DSPs and RN with medical care and observe health status changes.
• Pass medications and provide prescribed care under LPN scope.
• Schedule and attend medical appointments as directed.
• Document care per Springbrook policies and procedures.
