ATC Healthcare Services, Nassau County, is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to individuals with intellectual and developmental disabilities in Brookville, NY. The role involves administering medications, monitoring vital signs, and supporting care plans within a team environment.

Candidates must hold a New York State LPN license and BLS/CPR, with strong communication and organizational skills.

Formación

• Current, unrestricted New York State LPN license.
• Minimum of one year of LPN experience preferred.
• BLS/CPR Certification.
• Excellent communication, organizational, and critical thinking skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsabilidades

• Provide direct nursing care under supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.
• Monitor and document vital signs, health status, and changes in patient condition.
• Assist with developing and implementing individualized care plans.
• Maintain accurate and timely patient documentation.
• Provide health education and support to individuals and direct care staff.
• Assist individuals with medical needs while promoting dignity, independence, and quality of life.
• Ensure compliance with agency policies, New York State regulations, and HIPAA standards.
• Respond appropriately to medical emergencies and communicate effectively with the interdisciplinary healthcare team.
• Maintain a safe, clean, and therapeutic environment.
