Job summary

Valley Vista in Bradford, VT is seeking a Licensed Practical Nurse to provide direct nursing care to patients in detoxification or substance use treatment, monitor vital signs, and administer medications as ordered by a licensed provider.

You will document patient conditions, contribute to care plans under supervision, and collaborate with a multidisciplinary team to support MAT protocols and recovery goals.

Qualifications

• Current, active LPN license in Vermont.
• Graduate of an approved practical nursing program (certificate or associate).
• BLS certification (or ability to obtain upon hire).

Responsibilities

• Provide direct nursing care to patients undergoing detoxification or substance use treatment.
• Monitor and document vital signs, withdrawal symptoms, and patient condition.
• Administer medications as ordered by a licensed provider.
• Observe and report changes in patient condition to supervising RN or provider.
• Assist with wound care, specimen collection, and basic treatments.
• Contribute to assessment and care planning under clinical supervision.
• Implement nursing interventions and support continuity of care across shifts.
• Collaborate with counselors, case managers, and physicians.
• Monitor withdrawal using standardized tools (CIWA, COWS) if applicable.
• Support MAT protocols as directed.
• Provide patient education on recovery and medication adherence.
• Promote a therapeutic and trauma-informed environment.
• Maintain accurate, timely medical records and adhere to policies.
• Participate in quality improvement and incident reporting.
• Delegate tasks to nursing assistants within scope.
