Licensed Practical Nurse (LPN), Nursing Home Department

Job Type: Full-time, Day Shift

Under the supervision and delegation of the unit manager, provides direct patient care in accordance with applicable scope and standards of practice.

Core competencies include, but are not limited to:

• Assisting in patient assessment by collecting data for analysis under the direction of an RN.
• Performing plan of care interventions, including specimen collection and wound care. May administer medication. Documents patient care given.
• Assisting RNs with development of the plan of care.
• Helping patients perform activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitoring, documenting, and communicating patient's status and reactions to interventions with RNs, physicians, patients, and families.
• Advocating for the physical and emotional well-being of the patient.
