Lake Port Square is seeking a Licensed Practical Nurse (LPN) to join our Leesburg community. You will provide safe, resident-centered care and collaborate with a dedicated healthcare team to support independence and quality of life for residents.

Qualifications

• Current Florida LPN license in good standing.
• Minimum of 6 months nursing experience preferred.
• Experience in senior care, long-term care, rehabilitation, or skilled nursing is a plus.
• Strong clinical judgment and communication skills.
• Compassionate, resident-focused approach to care.
• Ability to thrive in a collaborative team environment.

Responsibilities

• Administer medications accurately and safely.
• Provide nursing treatments including wound care, catheterization, oxygen therapy, and other skilled nursing interventions.
• Monitor resident conditions and report significant changes.
• Maintain timely and accurate clinical documentation.
• Respond promptly to resident call systems and emergencies.
• Assist with admissions, transfers, and discharges.
• Collaborate with physicians and interdisciplinary care teams.
• Communicate resident status updates with families and healthcare providers.
• Promote resident dignity, independence, and quality of life.
• Participate in continuing education and in-service training programs.
