Lake Port Square in Leesburg, FL is hiring a Licensed Practical Nurse (LPN) to deliver compassionate, resident-centered care in a premier Life Plan Community.

As an LPN, you’ll administer medications, provide skilled nursing care, and collaborate with physicians and a multidisciplinary team to promote comfort, dignity, and quality of life for residents.

Qualifications

• Florida LPN license in good standing.
• Minimum of 6 months nursing experience preferred.
• Experience in senior care, long-term care, rehabilitation, or skilled nursing is a plus.
• Strong clinical judgment and communication skills.
• Compassionate, resident-focused approach to care.
• Ability to thrive in a collaborative team environment.

Responsibilities

• Administer medications accurately and safely
• Provide nursing treatments including wound care, catheterization, oxygen therapy, and other skilled nursing interventions
• Monitor resident conditions and report significant changes
• Maintain timely and accurate clinical documentation
• Respond promptly to resident call systems and emergencies
• Assist with admissions, transfers, and discharges
• Collaborate with physicians and interdisciplinary care teams
• Communicate resident status updates with families and healthcare providers
• Promote resident dignity, independence, and quality of life
• Participate in continuing education and in-service training programs
