Qualifications

• Current LPN/ RN license in the state of Kentucky.
• At least one year of LPN or RN experience in a long-term care setting is preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement and evaluate patient care plans.
• Prepares and administers medications and records them.
• Records patients' medical information and vital signs.
• Monitors equipment and interprets displays; initiates corrective action as needed.
• Provides bedside care and participates in discharge planning.
