Graceworks Enhanced Living in Ohio seeks a full-time Licensed Practical Nurse to join our group home team. This day-shift role (7a-3p, Monday–Friday) focuses on monitoring residents' health, administering medications, and coordinating care with physicians and staff.

You will document nursing observations, train staff on procedures, and ensure compliance with state and federal regulations while providing Gentle Teaching support to residents with developmental disabilities.

Qualificações

• Current Ohio LPN license to practice.
• Current CPR/First Aid and Pharmacology card.
• Valid driver’s license with good driving record.
• Experience with persons with developmental disabilities or dual-diagnosis in residential setting preferred.

Responsabilidades

• Monitor and meet health needs of residents in the group home; evaluate, record, and consult with physicians and staff.
• Administer medications and treatments; ensure nursing rules are followed.
• Train staff on medical procedures and disease management; document accordingly.
• Complete Nursing Summary quarterly and maintain accurate nursing notes.
• Ensure compliance with federal, state, and county regulations and IHP processes.
