Job summary

Graceworks Enhanced Living, affiliated with Graceworks Lutheran Services, seeks a full-time Licensed Practical Nurse for our Ohio-based group home. The role is a Monday–Friday day shift (7a–3p).

You will monitor residents, administer medications, and keep precise nursing records while complying with federal and state regulations. You will train staff on procedures and contribute to the IHP process, ensuring high-quality care.

Qualifications

• Current Ohio LPN license and active nursing status.
• CPR/First Aid and Pharmacology certification required.
• Valid driver’s license with reliable driving record.

Responsibilities

• Monitor residents' health and vital signs regularly and document findings.
• Administer prescribed medications and treatments per nursing rules.
• Maintain accurate nursing notes and quarterly summaries for each resident.
• Schedule and coordinate medical appointments and follow recommendations.
• Train and support staff on medical procedures, conditions, and medication management.
• Communicate status changes to the Home Manager and families as appropriate.
• Participate in IHP processes and ensure regulatory compliance.
