Job summary

Empath Health is seeking a dedicated LPN for overnight Continuous Care in Florida, working 8:00 PM – 8:00 AM with a 1:1 patient ratio. You will provide bedside clinical observations and prescribed care under the RN case manager, ensuring patient comfort and reporting changes promptly.

Must hold a Florida LPN license with at least 1 year in acute care; hospice LPN experience is preferred. Mileage reimbursement, competitive pay, and comprehensive benefits accompany this Full Life Care team role.

• Hospice LPN experience preferred.

What You'll Do

Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed. In collaboration with the RN, accepts and processes physician orders to maximize patient comfort. Continuous Care LPNs also assist Triage and Care Centers overnight when Continuous Care cases are pending.

What You'll Need

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
