Job description

Overview

Under RN supervision, the LPN, Continuous Care is responsible for providing nursing interventions and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.

Qualifications
  • A high school diploma or G.E.D. or an equivalent combination of education and experience.
  • Current LPN licensure in the state of employment.
  • 6 months experience as LPN preferred.
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.
  • For current Hope Employees: BLS should be obtained by January 1, 2025.
  • For current Capital Caring employees: BLS should be obtained by November 1, 2024.
  • Certificate of completion of a 30‑hour Infusion Therapy Course and an in‑house 8‑hour Infusion Class preferred if working in an inpatient unit.
  • Ability to communicate (written and verbal) in English.
  • Flexible schedule to meet the needs of patients.
  • Ability to use equipment with visual and auditory mechanisms.
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation.
  • Can work within their scope or practice per state regulations.
Competencies
  • Satisfactorily complete competency requirements for this position.
Responsibilities
  • Under the direction of an RN, provides nursing tasks to patients and families such as continuous care, dressing changes, and catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG).
  • Demonstrates clinical skills regarding application of company policies and procedures such as medication administration, symptom management, and patient/family relationships.
  • Observes and monitors patients for alterations in body systems, pain, and symptoms and reports findings to an RN.
  • Observes and reports signs of adverse reactions and responses to treatments and reports findings to an RN.
  • Reports findings to physicians, APRNs, and physician assistants following collaboration with an RN.
  • Performs venipuncture for blood specimens.
  • Performs Central Venous Line site observation/inspection; venous access device dressing, cap change, flushing, IV medication and fluid administration, IV pushes, and venipuncture. May not make any changes to infusion pump settings.
  • Documents per policy and procedure in the medical record.
  • During emergencies (e.g., hurricanes) may be required to report to a location designated by the company to ensure continuity of services.
  • Performs other duties as assigned.
  • Some locations may require cross training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing.
Additional Requirements
  • Reference to drug and alcohol testing after a conditional offer of employment and ongoing compliance with the drug‑free workplace policy.
  • Background screening through the Florida Care Provider Background Screening Clearinghouse for Florida affiliates, conducted after a conditional offer of employment and ongoing eligibility.
Compensation

Pay range: $21.64 – $31.17 per hour.
