We’re looking for an LPN to provide continuous nursing care and support as part of our Chapters Health Team. The role is under RN supervision, focusing on nursing interventions, teaching skills, and maximizing patient comfort.
Qualifications
- High school diploma or G.E.D. or an equivalent combination of education and experience.
- LPN licensure in the state of employment.
- 6+ months experience as an LPN preferred.
- Active BLS certification for healthcare professionals (American Heart Association or Red Cross). Current Hope employees must obtain by Jan12025; current Capital Caring employees by Nov12024.
- Valid driver’s license and automobile insurance. Reliable transportation required for patient visits.
- For inpatient assignment: certificate of completion of a 30‑hour Infusion Therapy Course and in‑house 8‑hour infusion class preferred.
- Ability to communicate in English.
- Ability to use equipment with visual and auditory mechanisms.
- Capacity to manage emotional stress when caring for terminally ill patients and families.
- Ability to perform essential functions: lifting patients/equipment, bending, pushing/pulling, kneeling.
- Can work within scope or practice per state regulations.
- Satisfactorily complete competency requirements for this position.
Responsibilities
- Under RN direction, provide nursing tasks such as continuous care, dressing changes, catheter changes as outlined in the interdisciplinary plan of care.
- Perform medications administration, symptom management, and maintain patient/family relationships.
- Observe patients for changes in body systems, pain, symptoms, and report to RN.
- Report signs of adverse reactions or treatment responses to RN, physicians, APRNs, and PAs.
- Perform venipuncture for blood specimens.
- Perform central venous line site observation, dressing, cap change, flushing and IV medication and fluid administration; not adjust infusion pump rates or bag changes.
- Document procedures according to policy and procedure in the medical record.
- Respond to emergencies (e.g., hurricanes) by reporting to the designated location ahead of schedule and staying overnight if required.
- Perform other duties as assigned.
- Cross‑train to Hospice House positions of equal licensure when needed.
Additional Information
Compensation: $23.80–$34.29 per hour.
