Job summary

Bremerton Trails Post Acute is seeking a Licensed Practical Nurse (LPN) to provide hands-on care for chronically ill and technologically dependent residents at our Washington, DC area facility. The position offers day and night shifts with a $2,500 sign-on bonus and close collaboration with RNs and an interdisciplinary team.

You will administer medications, manage complex treatments, and document care in accordance with state regulations and facility standards.

Qualifications

• Active LPN license in good standing.
• Experience with technologically dependent residents preferred.
• Strong clinical assessment skills and nursing judgment.
• Clear, effective communication with residents, families, physicians, and the care team.
• Ability to stand, walk, bend, and lift up to 25 lbs regularly (up to 100 lbs with assistance).

Responsibilities

• Deliver total care for chronically ill and technologically dependent residents, factoring in physical, psycho-social, and spiritual needs.
• Complete initial and ongoing resident assessments, distinguishing normal from abnormal findings and intervening per clinical standards and physician orders.
• Collaborate with residents, families, and the interdisciplinary team to build individualized care plans.
• Administer medications, including pain management, and provide IV care within your scope if IV certified.
• Perform treatments per physician direction – wound packing, irrigation, indwelling catheter care, and more.
• Monitor and report condition changes promptly to the RN, physician, family, and care team.
• Provide resident and family education tailored to individual needs, and document its effectiveness.
