Qualifications

• Current, unencumbered active LPN license in the state of Pennsylvania.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with accepted standards of nursing practice.
• Follow and implement individualized care plans developed by the Nurse Manager.
• Perform medication administration and prescribed treatments as per plan of care.
• Monitor residents’ conditions and report changes to the supervising nurse.
• Maintain accurate documentation in accordance with physician orders and regulations.
• Support a safe and respectful environment for residents and families.
