Job summary

Eagle Senior Living in Tennessee is seeking a licensed practical nurse to provide direct resident care and follow individualized plans of care. You will administer medications, perform assessments, and document observations daily in a collaborative team environment.

Qualifications

• High school diploma or equivalent (GED) required.
• Licensed Practical Nurse license in applicable state required.
• Current CPR certification required.
• At least 2 years of related work experience.
• Knowledge of medical terminology, medical records management, and clinical/diagnostic procedures.

Responsibilities

• Provide direct resident care services and follow each resident's individualized plan of care.
• Administer medications following standards of practice, state regulations, and required documentation.
• Perform resident assessments as required by state standards and facility policy.
• Observe residents for changes in condition and report immediately to the supervisor or Department Director.
• Document observations, communications, and daily care by the end of each shift.
• Complete and maintain individualized care plans; attend interdisciplinary care meetings.
• Check and record vital signs, weights, and blood sugars as assigned.
• Assist with supervision, coaching, and training of direct care staff.
• Support fall prevention and a restraint-free environment.
