Quality Center for Rehabilitation and Healing in Tennessee is seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality, resident-centered care in collaboration with the nursing team. You will monitor health, administer medications, maintain records, and support daily living activities while adhering to care plans and safety protocols. This role emphasizes teamwork and compassionate resident care throughout day-to-day operations.

Qualifications

• Current LPN license in the state where the facility is located.
• Strong communication and interpersonal skills.
• Compassionate, respectful approach to resident care.
• Ability to work collaboratively with the nursing and care teams.
• Long-term care or rehabilitation experience preferred.

Responsabilités

• Provide nursing care to residents within the scope of the LPN license.
• Monitor residents’ health and report changes in condition to the appropriate nursing staff.
• Administer medications and treatments as assigned and in accordance with care plans, facility policies, and applicable requirements.
• Document care provided and maintain accurate resident records.
• Communicate with residents, families, and members of the care team.
• Support residents with daily care needs as appropriate.
