Bradford Hills Nursing & Rehabilitation Center is seeking a Licensed Practical Nurse (LPN) to provide direct patient care, assess residents, administer medications, and support treatment plans within an interdisciplinary team.

Responsibilities

• Provide direct patient care to residents, including assessments, medications, treatments, and procedures
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and detailed records of patient treatments, medications, and progress
• Report patient concerns and changes to the registered nurse or supervisor
• Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
• Collaborate with other departments to ensure seamless care transitions and coordination
